Billing and Payment
Unless other arrangement are made, payment is due as follows:
- for individual sessions, pay each visit. New client’s first session must be paid in full before the session commences.
- for any written reports requested by the client for medical, school, or legal purposes, are prorated and charged to the client at standard fee, prorated to the nearest quarter-hour.
- crisis phone consultations are charged at the standard fee, prorated to the nearest quarter-hour. Please note that insurance carriers do not reimburse for telephone consultations.
- The fee for Individualized Education Plan meetings or other treatment related meetings will be charged according to the hourly fee.
Payment is due at the time of service. Acceptable payment methods are Cash (exact amount only), Check, and PayPal (Master Card and Visa).
Cancelation Policy
If you need to cancel or reschedule an appointment, please notify me as soon as possible, at least 48 hours in advance. The full session fee will be charged to you for missed sessions not canceled at least 24 hours in advance. This policy is necessary because a professional time commitment is set aside and held exclusively for you.
Running Late? Here’s how it works:
Should I be late, the time is made up. If you are late, the sessions still ends on time. You are responsible for the full session fee.
Insurance
Due to hypnotherapy being short term, I do not take insurance at this time.
Pre-Appointment Information
Be mindful that on your first appointment there are two forms to be filled out before we start. Please arrive 10-15 minutes early so you have enough time to finish the forms so we can begin on time.
